Explanation

This branch is easy to miss because the operator isn’t necessarily isolated or unexposed to new ideas — they may read plenty, talk to plenty of people, and still be running [Stale Thinking], because the problem isn’t intake, it’s revisiting. Assumptions set two years ago about pricing, staffing, or vendor terms never get re-examined, not because the operator refuses outside input but because nobody built a discipline to force the look-back.

The three named correctives attack this from different angles on purpose. [Reality Check] forces it on the calendar regardless of triggers.
